We have all been there- you just don’t fit with your physician or midwife and want to switch. You are concerned because you do not want to cause any hurt feelings and are unsure how to go about it. We understand and want to help you find a provider who meets your needs. If you find yourself in this position, here are some tips to make a seamless switch:

  • Research and find another provider that you would like to transfer your care to. We recommend reading the biographies of the providers on our website. Also ask family and friends who they recommend.
  • If you choose to stay with Circle of life, just schedule your next appointment with the provider you have chosen. It’s that simple! We have many different providers to ensure our patients find one that works well with them and their needs.
  • If you choose to go elsewhere, please fill out a “Request for Records” form. This is located on our website or at the front desk. We can then send your records over to your new physician.
